How federally funded HCPs for seniors’ physio works.
Did you know that he Australian government subsidises Home Care Packages (HCPs) for seniors like you, Mum or Dad to support your complex needs?
This financial assistance for at-home services enables seniors to stay at home for longer and enjoy living independently. And they can be used for physiotherapy.

- Eligible seniors get approved for a Home Care Package (HCP) and sign up with a HCP provider – a 3rd party company that manages the HCP budget.
- The HCP provider will assign a manager to assess your needs and refer appropriate service providers such as our team at TPC.
- The HCP manager will coordinate physio assessment, goal plans and treatments with TPC. (Alternatively, you can communicate directly with TPC and we can help you to get started.)

Once we receive a referral, we reply within 2 hours to schedule a call with our new senior client and/or their HCP Manager.
Within 24 hours, we’ll arrange a welcome call for you and/or your HCP Manager at a convenient time.
A TPC physio will visit you/your Mum or Dad for the 1st appointment. They’ll listen closely to understand all pain and/or health problems.
Over the first 1-2 appointments, your physio will complete an assessment of your health, fitness and/or injury.
Then, they’ll create a one-page plan of the health goals we agree are most vital to work on together.
We share the agreed goal plan with our client and their HCP manager +/- the client’s family.
Sharing your goal plan creates a small team that’s set for success. We’ll have all the information on what we’re working on, for how long, and the costs involved.
Your physio will guide you through the recommendations and/or exercises. This ensures you’ll get the best results and work towards your health and fitness goals.

What is a HCP?
The term “home care package” refers to an amount of money allocated by the federal government to an older person to provide services which will enable them to continue to live independently. The amount of money provided depends on the person’s needs, as assessed by an independent assessment agency. There are certain packages older adults are eligible for, from basic care needs (Level 1) through to high care needs (Level 4). They can use these funds towards services such as social support, well-being services, home modifications as well as allied health services such as physiotherapy.
How does a HCP work?
Usually once a senior has been approved for a HCP, they will sign up with a provider to help manage their funds on their behalf. The provider will assess the clients care needs and then source third parties to help provide the care like TPC.
When a client has a HCP, we would obtain the HCP case manager details and call them on behalf of the client to introduce our services. The HCP manager then sends us an agreement for our services or approves the physio sessions for this client.
When a HCP manager sends through a referral to us,we will reply within the first 2 hours, we will then aim to have this client booked within 24 hours and welcome them into The Physio Co family.
Our physio will then attend the initial appointment and listen closely to what pain/ health problems our client has along with conducting an assessment.
After the initial assessment the physio will create a detailed goal plan that includes:
- A specific meaningful goal the client is wanting to achieve
- What the physio and the client will be working on together to reach this goal
- How many sessions the physio recommends along with a review date of when they hope the client will achieve the goal by.
We then send this goal plan to the clients HCP manager for approval, that way they know what we are working on, how long we aim to work on it for and understand the costs involved.
Then our caring physio gets to work with our client on their goal!
When the review date comes up, we typically celebrate with our client and their family in smashing the goal! We also review the client’s progress for further physio as needed.
Once approved, we then send an invoice to the HCP company for payment.
Responsive and Proactive: We respond within 2 hours, and book appointments within 24 hours. We always remain in contact regarding a client’s condition, and if appointments have been cancelled by the client.
Clear Communication: Creating and sending our physio’s recommendations and client Goal Plan to the HCP manager for approval helps them manage budget and expectations. No appointments go ahead without this approval.
